What is the rule of thumb for cable runs (max distance) using serial data between devices and is there a preferred cable to use?

Because there are so many variations, there is no single rule of thumb to apply. What distances are you looking for? What voltage (e.g., 3.3V, 5V, 12V, true RS232 voltages)?
