L293D blown?

How do i check if i have blown my H-bridge motor driver?

I have verified with my multimeter that the inputs are sending the right voltages from my uln darlington IC.
All connections are correct, just no motor movement, from a simple "let pins = %10000000"

motor specs are
.5A @ 12v

Driver is using a separate 12v power supply
Darlington IC and Picaxe 28x1 are using same 5v power supply
all are connected to a common ground.

Can you post some sort of circuit diagram to show exactly how everything is wired ? It's hard to second guess what you have and hence what could be wrong.
